This session will walk through the definitions of several types of errors and how to teach patient safety from error recognition to analysis of error to facilitating error discussions in large multi-specialty group settings. Utilizing a case-based format, the attendee will understand the significance of error identification and just in time debriefing and event reporting. We will then apply tools discussed in previous sessions to aid with error analysis involving learners. Finally, we will discuss strategies for a successful MMI conference with a strong emphasis on psychologically safe facilitation and communication skills.

1. Apply a framework for selecting cases and developing an educational Morbidity, Mortality, and Improvement (MMI) conference.
2. Model communication strategies for engaging faculty, learners, and multidisciplinary team members in discussions of patient safety events in different clinical and educational settings.
3. Outline techniques to ensure psychological safety when facilitating discussions surrounding cognitive and implicit biases. Participants will understand strategies for role modeling transparency in clinical decision-making.
4. Explain the role of “Just Culture”, including System, Cognitive and Diagnostic Errors in the framework of MMI education.
