The second module of this two part series will provide and overview of the Society of Hospital Medicine's Facilitation of Anticoagulation for Safer Transitions (SHM FAST) Program. This module is a supplement to the SHM FAST Implementation Guide.

Learning Objectives

After completing the module, the participant will demonstrate the ability to:

  1. Describe the objectives and potential benefits of the SHM FAST program
  2. Describe the differences between the comprehensive, intensive FAST bundle and the standard FAST bundle
  3. Review the process to assess baseline performance and program success
